The University of Pennsylvania Health System has created and entered a joint venture with Evergreen Nephrology to improve kidney outcomes while containing costs of care, according to a press release.
As part of The University of Pennsylvania Health System’s (UPHS) value-based population health strategies, the partnership contracts with CMS to provide care for Medicare patients with kidney disease.
